About this template

Bringing a subcontractor onto a job means taking on their paperwork as well as their labour. This form collects both halves at once. The subcontractor gives their legal and trading name, registered address, company or tax number, VAT status and the named person responsible for the work on site. The engagement itself is described precisely: the main contract or project it sits under, the works package, the site address, the start and finish dates, and whether they are supplying labour only or labour and materials. Rates are captured in a form your finance team can use directly — day rate or price for the package, currency, retention percentage, payment window and the invoicing route. Compliance is where most onboarding stalls, so evidence is requested inline: public liability and employer's liability insurance with insurer, cover level and expiry date, relevant trade qualifications or cards, a right-to-work confirmation and, where the work calls for it, a risk assessment and method statement upload. Health and safety questions confirm who provides equipment and PPE and who supervises. The subcontractor signs the declaration confirming the information is accurate and current, and every certificate is stored against the submission for the audit that eventually asks for it.
