Who this template is for

Service Agreement Form is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in contracts & agreements.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 37 questions across 5 screens.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 28 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It records a signature you can keep as proof of agreement.
  • It pins the request to a specific date.
  • It captures a full postal address.
  • It captures a numeric value you can filter and sort on.
  • Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 5 pages keeps each screen short and shows respondents how much is left.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 4 conditional rules ship with it.

  • Features conditional validation: if “Does the service include an availability commitment?” is Yes, “Availability target” becomes mandatory before the form can be sent.
  • Features advanced show logic: if “How is the service priced?” is Usage-based, the form dynamically exposes “Describe the usage bands”. Otherwise that question never appears.
  • Features conditional validation: if “Does the contract renew automatically?” is Yes, “Notice period to prevent renewal” becomes mandatory before the form can be sent.
  • Features advanced show logic: if “Will the supplier process personal data?” is Yes, the form dynamically exposes “Describe the personal data processed”. Otherwise that question never appears.
  • All 4 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

An ongoing service arrangement fails quietly when nobody wrote down what 'ongoing' means. This form fixes the terms that actually get argued about later. Both parties are identified by legal name, registered address and the person who is authorised to sign. The service is described in full — what is delivered, how often, where, and which hours it covers — with a separate question for anything explicitly excluded, because exclusions are what stop scope creep. Service levels are captured as structured answers: response time, resolution target, availability commitment and the reporting cadence the client should expect. The commercial section records the fee model, billing frequency, the review or uplift mechanism, and the payment window, with an optional volume or usage band where the price moves with consumption. Term questions cover the start date, initial term, renewal behaviour, notice period and whether a termination fee applies. Data-handling and confidentiality questions appear where the service touches personal data or systems, and the responsible contact on each side is recorded. Both signatories sign and date on screen, and the complete answer set is exported as a single PDF for the contract file.

Privacy & compliance

This form takes money. Card details are handled by the payment provider and never stored in your response data — you keep the order details, not the card number.

Frequently asked questions

Does it cover service levels?

Yes — response, resolution, availability and reporting cadence are separate structured questions rather than free text.

Can it handle usage-based pricing?

Choosing usage-based pricing reveals a band description question so the pricing mechanism is written down.

What about data protection?

If the supplier processes personal data, the form asks what data and why, which is the starting point for a processing schedule.

Can I reuse it for every client?

Copy it once, set your standard options, and reuse the same published link for every new agreement.

Is the Service Agreement Form template free to use?

Yes. You can preview and test Service Agreement Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

What does the Service Agreement Form template ask for?

It asks 37 questions across 5 pages, 28 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.

Can I edit the Service Agreement Form form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic layout, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this contracts & agreements form work?

4 conditional rules ship with the template: Features conditional validation: if “Does the service include an availability commitment?” is Yes, “Availability target” becomes mandatory before the form can be sent. Features advanced show logic: if “How is the service priced?” is Usage-based, the form dynamically exposes “Describe the usage bands”. Otherwise that question never appears. Features conditional validation: if “Does the contract renew automatically?” is Yes, “Notice period to prevent renewal” becomes mandatory before the form can be sent. All rules are editable in the Logic tab.

Where do responses to the Service Agreement Form form go?

Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.

Can I embed the Service Agreement Form form on my own website?

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.

Is the signature on this form legally usable?

The template captures a drawn signature and stores it with the submission, along with the timestamp, so you keep evidence of who agreed and when. Whether that is sufficient depends on your jurisdiction and the agreement itself.
