A statement of work only protects a project if someone with authority actually approved it, and you can show when. This form is the approval step. It records the SOW reference, version and the master agreement it sits under, so the paperwork chain is unambiguous. The scope section captures objectives, deliverables, acceptance criteria and assumptions — acceptance criteria being the part that decides whether an invoice is payable. Dates cover the start, the delivery milestones and the planned completion, with a dependency question for anything the client must provide before the clock starts. The commercial section records the total value, the pricing model, the payment schedule and any not-to-exceed cap, plus a change-control note explaining how variations will be priced. Reviewers are named individually: the delivery owner, the commercial approver and, when the value crosses your threshold, a second authoriser whose approval question only appears for higher-value work. Each approver records a decision of approve, approve with conditions, or reject, with conditions and reasons required where they apply. The signed approval, the version number and the timestamp are stored together, which is exactly what an audit later asks for.
