Tournament organisers need entries that are complete enough to seed a draw and safe enough to satisfy an insurer. This entry form captures the competitor's name, date of birth, club or unaffiliated status, and governing body membership number where the competition requires one. The entry category is selected from your event's divisions, with weight, age or grade brackets and a ranking or handicap where relevant, plus doubles or team partner details when the format allows. Eligibility questions confirm qualification results, previous placings and whether the competitor is currently serving any suspension. The medical block covers conditions relevant to competition, current injuries, medication that may fall under anti-doping rules, and an emergency contact who will be at the venue or reachable on the day. The waiver sets out the assumption of risk, agreement to abide by the rules and the referee's decisions, media consent for results and photography, and refund terms. Entries arrive ready to seed, with waivers already signed and timestamped.
