A tryout is a selection event, not a registration, and the form should collect what a coach needs to run one. Where the athlete is under 18 it is completed by a parent or guardian. The first page covers the athlete, date of birth and age group, the tryout session they are attending, the position or event they are trying out for with a second choice, current and previous clubs, how long they have played, and their level. It asks about representative experience and any accolades, briefly, because coaches read those in seconds.
