About this template

Leagues lose officials to admin: someone volunteers in August, is asked for a certificate in September and appointed in November, by which time they are refereeing elsewhere. This form collects everything an appointment needs in one pass. The first page covers the applicant, the sports and levels they can officiate, years of experience, current qualification with its grade and expiry date, whether they are registered with a governing body and their number, and any level they are working towards. The second page is the practical side: availability by day and time, how far they will travel, whether they drive, which venues they can reach, whether they can officiate midweek evenings, and whether they are available for tournaments and finals. It asks about background checks required for working with young people, references, and payment or expenses details. Two questions matter more than they look: whether they have any conflicts of interest with local clubs, and whether they would like mentoring, which is how new officials stay.
