About this template

Almost every payment dispute in field service traces back to the same gap: work started on a verbal yes. This form closes it. It restates the scope in writing, names a figure, sets a validity date on that figure, and takes a signature against agreed payment terms — the four elements that make an estimate enforceable in practice as well as in principle. Payment terms are a deliberate three-way choice, and selecting deposit plus balance requires the deposit amount immediately rather than leaving it to an invoice. Estimates above a threshold reveal a required second approver email, which is how larger jobs stay inside a customer's own internal authority limits without you having to ask who signs. Kept alongside the original work order and the completion sign-off, it gives a three-document trail from request to closure. Send it as a link the customer signs on a phone; a photographed paper form does the same job far less reliably.
