- Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
- Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
- Financial disclaimer present — passingThis form does not provide financial, tax or legal advice. Any figures shown are indicative only and are not an offer of credit or a guarantee of terms. Please seek advice from a regulated adviser before making a decision.

Who this template is for
Work Order Form is built for builders, electricians, plumbers and contractors who need to scope a job and price it without a site visit for every enquiry.
- Builders, electricians, plumbers and contractors working in home services & trades.
- Teams who need to scope a job and price it without a site visit for every enquiry without writing code or paying for a custom build.
- Anyone replacing quoting blind and travelling out for work that never lands with one structured record per enquiry.
- Respondents are homeowners and site managers — the form asks them 15 questions across 2 screens.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of quoting blind and travelling out for work that never lands and turns each submission into a record your team can act on immediately.
- 7 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It accepts a document or photo upload as evidence.
- It pins the request to a specific date.
- It captures a full postal address.
- It captures a numeric value you can filter and sort on.
- Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.

- 2Rename or delete any question, and change what is required. Nothing here is fixed.
- 3Open Notifications and add the email addresses that should be alerted on each submission; add an auto-reply to the respondent if the form collects an email address.

- Features advanced show logic: if “Category” is Other, the form dynamically exposes “Describe the trade needed”. Otherwise that question never appears.

- All 4 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
A work order exists to make a job schedulable without a phone call, and most fail because they collect a description and nothing else. This one collects the four things a dispatcher needs before assigning anyone: what the problem is, exactly where it is, how urgent it really is, and who has agreed to pay for it. Trade category is a dropdown so jobs sort to the right engineer rather than the first available one, and choosing anything outside the list requires a description of the trade needed. Priority is three-way, and urgent reveals a required callback number so a genuine emergency does not sit in an inbox. Access instructions and a photo are optional but transform first-time fix rates — a picture of the boiler model saves a return visit for parts. The authorisation page keeps the purchase order, spending limit and approver on the same record, which is what stops a completed job becoming an invoice dispute.

How the form changes as it's filled in
This template ships with 4 conditional rules, grouped into 4 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “Priority”
- Reveals questions
If “Priority” is Urgent, the form reveals “Callback phone number”.
- Makes answers required
If “Priority” is Urgent, “Callback phone number” becomes required before the form can be submitted.
Driven by “Category”
- Reveals questions
If “Category” is Other, the form reveals “Describe the trade needed”.
- Makes answers required
If “Category” is Other, “Describe the trade needed” becomes required before the form can be submitted.
Set it up step by step
- 1
Make the location a real address
"Upstairs office" costs an engineer twenty minutes; a full address with access notes does not.
- 2
Define what urgent means
Write the definition into the priority help text — without it, everything arrives urgent.
- 3
Insist on the spending limit
An agreed ceiling on the order is what lets an engineer buy a part without a second approval call.
- 4
Close every order with a sign-off
Pair it with the Job Completion Sign-Off Form so raised and closed jobs reconcile.
Mistakes to avoid
- Letting anyone mark a job urgent with no definition, which destroys the scheduling value of the field.
- Collecting the approver's name but not their email, so authorisation cannot be evidenced later.
- Skipping the photo, then sending an engineer who has to return for the right part.

Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses

Frequently asked questions
How is this different from a repair request?
The Facility Repair Request is for tenants and staff reporting a fault; a work order is the authorised instruction to carry out the work.

Does it handle recurring maintenance?
Use the Preventive Maintenance Checklist for scheduled inspections; this form is for reactive work.
Is the Work Order Form template free to use?
Yes. You can preview and test Work Order Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.
