How is this different from a feedback survey?

Feedback is private research to improve the business. This collects reviews intended for publication, so it captures a headline, a star rating and explicit permission to publish.

Can I publish a review without permission?

Do not. Publish only what the permission question allows, and re-ask if you want to use a quote in a different way, such as a paid advert.

Should the photo be required?

No. Photos lift conversion but requiring one cuts your response rate sharply — keep it optional and thank the people who add one.

What about negative reviews?

Collect them, reply privately and fix what you can. Publishing a few critical reviews alongside good ones makes the whole set more credible.
