About this template

A review is only an asset if you are allowed to publish it, which is what separates this form from ordinary feedback. It collects the reviewer's name, what they are reviewing, a one-to-five star rating, a headline, the review itself, whether they would recommend you, and — the question that makes the rest usable — how their name may be shown: in full, first name only, or not published at all. An optional photo of the product in use gives marketing something better than stock imagery. Feedback surveys are for improving the business and are usually confidential by default; this form assumes the opposite, that the answers are heading for a website, and gathers consent accordingly. Two rules shape the ending: declining publication reveals a note thanking the reviewer and confirming the comments stay private, while a rating of four or five reveals a friendly prompt to leave the same review on Google or your marketplace listing, where it does the most good. Keep the headline question — it is the line that gets quoted.

Can I publish a review without permission?
