Who this template is for
Product Recall Response Card is built for teams who want a conversational, one-question-at-a-time flow who need to keep completion high by asking one thing per screen.
- Teams who want a conversational, one-question-at-a-time flow working in card forms.
- Teams who need to keep completion high by asking one thing per screen without writing code or paying for a custom build.
- Anyone replacing long scrolling forms abandoned halfway with one structured record per enquiry.
- Respondents are anyone answering on a phone — the form asks them 13 questions across 2 screens.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of long scrolling forms abandoned halfway and turns each submission into a record your team can act on immediately.
- 9 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It accepts a document or photo upload as evidence.
- It captures a full postal address.
- It captures a numeric value you can filter and sort on.
- Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
- Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 3 conditional rules ship with it.
- Features advanced show logic: if “Do You Still Have the Product?” is Yes, the form dynamically exposes “Product Label Photo”. Otherwise that question never appears.
- Features advanced show logic: if “Requested Next Step” is Replacement, the form dynamically exposes “Replacement Delivery Address”. Otherwise that question never appears.
- Features advanced show logic: if “Do You Need Help Completing This Response?” is Yes, the form dynamically exposes “Help Needed”. Otherwise that question never appears.
- All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

Questions about this shared form
Should customers submit one card for every unit?
Use one card for units with the same model and batch, with the total entered in Number of Affected Units. For serial-number-based recalls, configure the card for one unit per response or add a structured way to collect each serial number.
What if the customer cannot read the product code?
Accept Unreadable or Unavailable and route the response for review. An existing label photo can help, but it should not be mandatory or require handling the product.
Does submitting this card approve a remedy?
No. The card records product details and the customer's requested next step. Confirm eligibility and the available remedy separately, using the official recall notice.
Can this replace our general contact form?
Keep recall responses separate so they reach the right team. Use the Say Hello Contact Card for ordinary questions, and direct recall-related contacts to this dedicated card.
Is the Product Recall Response Card template free to use?
Yes. You can preview and test Product Recall Response Card on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.
