About this template

A recall response card should answer three operational questions: which product the customer has, how many units are involved, and how the recall team can reach them. Publish one version for each recall notice, with the notice reference prefilled wherever possible. Put a link to the official notice above the form so customers can check affected models and follow the published instructions before submitting. Use the product identifiers printed in that notice. If the recall depends on a batch code rather than a serial number, change the label and explain exactly where to find it. Allow customers to enter “Unreadable” rather than guessing. Keep photos optional, and never ask someone to handle a product just to complete the card. Limit the requested next steps to remedies actually available for this recall. A preference is not an approval: tell customers that the team will check the product details before confirming a remedy. Show the delivery address only for replacement requests; if it is missing, collect it after review rather than blocking the initial response. Route submissions to a recall-specific queue, not a general sales inbox. Match repeated responses using the notice reference, product identifier, and contact details, while preserving legitimate claims for multiple units. Accept Unreadable or Unavailable and route the response for review. An existing label photo can help, but it should not be mandatory or require handling the product.
