Who this template is for

Breakfast Order Form is built for restaurants, hotels and venues who need to take bookings with party size, timing and special requests.

  • Restaurants, hotels and venues working in restaurants & hospitality.
  • Teams who need to take bookings with party size, timing and special requests without writing code or paying for a custom build.
  • Anyone replacing handwritten booking notes and lost dietary requirements with one structured record per enquiry.
  • Respondents are guests and diners — the form asks them 11 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of handwritten booking notes and lost dietary requirements and turns each submission into a record your team can act on immediately.

  • 6 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It captures a phone number for urgent follow-up.
  • It allows multiple selections without free text.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “Main” is Omelette, the form dynamically exposes “Omelette fillings”. Otherwise that question never appears.
  • Features conditional validation: if “Main” is Omelette, “Omelette fillings” becomes mandatory before the form can be sent.
  • Features conditional validation: if “Payment” is Room charge, “Room, table number or pickup” becomes mandatory before the form can be sent.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

The paper breakfast card hung on a bedroom door handle is a good system with one flaw: somebody has to read the handwriting at six in the morning. This form does the same job digitally and adds the parts that go wrong on paper. Page one is the order — main, fillings, egg style, sides, drink and dietary notes — with the filling question appearing only when an omelette is chosen, so the card stays short for everyone else. Page two is the logistics the kitchen actually needs: who it is for, a phone number, the room, table or pickup point, the serve time, and how it is being paid. It differs from the Catering Order Form, which quotes a whole event with headcounts and lead times; this one is a single service, ordered the night before or the same morning. Choosing a room charge requires the room number, because a folio posting without one is a write-off.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Main

  • Reveals questions

If “Main” is Omelette, the form reveals “Omelette fillings”.

  • Makes answers required

If “Main” is Omelette, “Omelette fillings” becomes required before the form can be submitted.

Driven by “Payment

  • Makes answers required

If “Payment” is Room charge, “Room, table number or pickup” becomes required before the form can be submitted.

Set it up step by step

  1. 1

    Put your menu in

Replace the mains, sides and drinks with your own menu, in the order you want guests to read it.

  2. 2

    Set your cut-off

Say in the cover text when orders close for the next morning — most kitchens use 22:00.

  3. 3

    Limit the serve times

Restrict the time question to your service window so nobody orders breakfast for 04:30.

  4. 4

    Match your payment routes

Keep only the payment options you offer; add a card payment question in the builder if you take money up front.

  5. 5

    Get it to the kitchen

Send submissions to the kitchen printer inbox and to the duty manager, then sort by serve time.

Mistakes to avoid

  • Taking the order without a serve time, which sends the whole night's orders to the pass at 08:00.
  • Offering an omelette with no filling question, so the kitchen guesses and the guest sends it back.
  • Allowing a room charge without a room number, which leaves the front desk unable to post the bill.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Print or route orders to the kitchen sorted by serve time, mark room charges against the folio the same morning, and keep a week of orders to see which mains actually sell before the next menu change.

Works in both layouts

These pages cover the same subject ground as the breakfast order form, matched on the words people actually search for.

Privacy & compliance

This form takes money. Card details are handled by the payment provider and never stored in your response data — you keep the order details, not the card number.

Frequently asked questions

Can guests pay on the form?

As shipped it records how they intend to pay. Add a Product List question in the builder and you can take card payment at the point of ordering.
