The client chooses the session type — one-to-one, pair, small group or a block of sessions — and the sport or discipline, then states their current level honestly against clear descriptions rather than vague labels. Goal questions ask what they want to improve, what has been holding them back, and whether they are preparing for a specific fixture, trial or event with a date. Availability is captured as days and time windows plus a preferred venue where you coach at more than one site. A short physical section notes injuries, recent surgery and anything that limits movement, so a coach can adapt rather than discover mid-drill. Equipment questions confirm what the client will bring. The booking closes with the cancellation window, a reminder preference and, if you use it, a payment step for the session or block. Every booking arrives ready to plan against.
