The second page is the history: cardiac symptoms including chest pain, fainting or unexplained breathlessness on exertion, family history of sudden cardiac death before 50, previous concussion with dates and recovery, seizures, asthma, heat illness, previous fractures or joint injuries, current medication, allergies, and the date of the last physical.
