This acknowledgement form creates that record for anyone who touches sensitive information: employees, contractors, interns, board members and visitors. The signer identifies themselves and their relationship to the organisation, names their team or engagement, and states the date their access begins. The policy version and effective date are recorded on the form itself, so a later revision does not quietly inherit an old acceptance. The obligations are then acknowledged individually rather than in one blanket tick — confidential information, personal data, intellectual property, systems access, use of personal devices and post-departure obligations each get their own required confirmation, which is far more defensible than a single checkbox. A device question captures whether the signer will access data on personal equipment, opening follow-up questions about encryption and device return. Where the signer is a contractor, additional questions capture the engaging company and the end date of access. The signature and timestamp complete the record, and the whole thing exports as a PDF for the personnel or supplier file.
