About this template
Paper permission slips come back late, damp and half-signed, and the missing ones are always discovered on the morning of the trip. This digital slip fixes that. It states the trip, destination, date, departure and return times and the cost, then asks the parent to consent or decline explicitly, so a non-response is visibly different from a refusal. Child details include the class or group and, importantly, any medical condition, allergy or medication relevant to the day, with a required details box when the answer is yes. Transport consent is separated by mode — coach, minibus, public transport or walking — and any travel sickness or seating needs are noted. Two emergency contacts who will be reachable during the trip hours are required. Consent covers photography during the trip, emergency treatment where a parent cannot be reached, and any activity-specific risk such as water or heights. Parents sign on screen and you get a live list of who has returned their slip.
