About this template

Internal moves fall apart in the gap between two managers: one has agreed to take someone, the other has not agreed to release them, and nobody wrote down a date. This form closes that gap. It records where the employee is now — role, team, site, manager, time in post — and where they want to go, including whether they are applying to a specific advertised vacancy or expressing interest in a team. It asks for the reason and the skills that transfer, since an internal application still needs a case. Then it deals with release: the earliest date the employee could move, the notice their current team needs, what is in flight, and who could pick it up. Pay, hours and location changes are asked about openly rather than assumed to stay the same, and a relocation question appears only when the site changes. The final page is two sign-offs with dates: the releasing manager and the receiving manager. Until both are recorded the move is a conversation, not a plan.
