About this template

The most common probation outcome is no outcome at all: the end date passes, nobody signs anything, and the employee is confirmed by accident. This form exists to force a decision before that date. It records the probation start and contractual end date at the top, so the deadline is visible while the form is being filled in, and asks which review this is — an interim check or the final one. The manager then scores the areas probation is actually about: the work itself, reliability, working with others, following process, and progress against the objectives set at the start, each with a short evidence box rather than a rating alone. Crucially the form asks what support and feedback the employee was given, because an outcome that cannot show the employee knew about a problem is difficult to defend. It closes with one decision — pass, extend with a new end date and specific conditions, or do not confirm — plus the employee's own comments and a countersignature. An extension cannot be saved without a new date and conditions, which is what stops an extension becoming a slow drift.
