Compliance checklist
- Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
- Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
- Legal text not required — passingThis template collects neither health nor regulated financial information.
- Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.
What happens after submit: nothing is stored in this preview. On your own copy, every response triggers an instant email notification and lands in your response dashboard, ready to export or forward.

Who this template is for
Job Interest Form is built for HR, people ops and hiring managers who need to run hiring and internal people processes on one auditable record.
- HR, people ops and hiring managers working in human resources.
- Teams who need to run hiring and internal people processes on one auditable record without writing code or paying for a custom build.
- Anyone replacing CVs, requests and approvals scattered across inboxes with one structured record per enquiry.
- Respondents are candidates and employees — the form asks them 11 questions across 1 screen.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of CVs, requests and approvals scattered across inboxes and turns each submission into a record your team can act on immediately.
- 6 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It accepts a document or photo upload as evidence.
- It pins the request to a specific date.
- It allows multiple selections without free text.
- Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.
How to use this template
From copy to live form is a few minutes of work, and every step is reversible.

- 2Rename or delete any question, and change what is required. Nothing here is fixed.
- 3Open Notifications and add the email addresses that should be alerted on each submission; add an auto-reply to the respondent if the form collects an email address.
- 4Check the upload limits on the file question so respondents can attach what you actually need.
- 5Publish it, then either share the link directly or paste the embed snippet into your site — the embedded form resizes to fit and loads no marketing or advertising trackers.
- 6Watch responses land in Submissions, where you can filter, label and export them to CSV or PDF.
Logic and conditions blueprint
Exactly how this form behaves as it is answered — 3 conditional rules ship with it.
- Features advanced show logic: if “Is it OK to keep your details on file?” is No, the form dynamically exposes “What happens to your details”. Otherwise that question never appears.
- Features advanced show logic: if “Employment type” contains Seasonal, the form dynamically exposes “Which season?”. Otherwise that question never appears.
- Features conditional validation: if “Employment type” contains Seasonal, “Which season?” becomes mandatory before the form can be sent.
- All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
Most careers pages only work when a vacancy is live, which means every good candidate who looks on the wrong week is lost. A job interest form fixes that: it takes nine questions, sits permanently on the site, and builds a pool you can search the day a requisition is approved. It captures name, email and phone, the area of work someone is interested in, the employment types they would consider, an earliest start date, an optional CV upload, a short piece of free writing and — importantly — permission to keep the details on file. It differs from the Employment Job Application Form in both depth and timing: that one is tied to a specific advertised role and collects referees, right-to-work answers and a full history, because a hiring decision follows. This one is pre-vacancy. It exists to answer 'what would it take to work here one day?' without pretending a shortlist is imminent. Two rules keep it honest: declining consent to be kept on file reveals a note explaining that the entry is deleted after the reply, and selecting seasonal work opens a question about which season, which is the only detail a seasonal recruiter actually needs up front.

How the form changes as it's filled in
This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “Is it OK to keep your details on file?”
- Reveals questions
If “Is it OK to keep your details on file?” is No, the form reveals “What happens to your details”.
Driven by “Employment type”
- Reveals questions
If “Employment type” contains Seasonal, the form reveals “Which season?”.
- Makes answers required
If “Employment type” contains Seasonal, “Which season?” becomes required before the form can be submitted.
Set it up step by step
- 1
Link it from every job page
Put 'no suitable role today?' at the foot of each vacancy so near-misses land in the pool instead of leaving.
- 2
Match areas of work to your org chart
Replace the placeholder areas with your real teams so the pool can be filtered the way you actually hire.
- 3
Set a retention period
Twelve months is a common default. State it on the cover and honour it — this is the question candidates notice.
- 4
Reply to everyone
A two-line acknowledgement keeps the pool warm and stops the same person applying four times.
Mistakes to avoid
- Running it like a full application — a CV and eight questions is the ceiling before people abandon a form for a job that does not exist yet.
- Keeping details on file indefinitely without asking permission, which is both rude and, in most jurisdictions, unlawful.
- Never contacting the pool, so when a vacancy opens the register is a list of people who took other jobs eighteen months ago.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Tag each entry by area of work and employment type, review the pool whenever a requisition is approved, and delete or re-consent anyone who has been on file for twelve months.
Works in both layouts

Frequently asked questions
How is this different from a job application form?
An application is tied to an advertised vacancy and collects everything needed to decide. This is pre-vacancy: a short expression of interest that keeps a candidate reachable until a role exists.
Should the CV upload be required?
No. Requiring a CV turns a two-minute expression of interest into a job application, which is exactly what suppresses submissions.
How long can we keep the details?
Only as long as the person agreed to. Say the period plainly on the form and delete or re-ask when it expires.
Can we use it for internal moves?
Yes — duplicate it, restrict the link to staff, and swap the CV question for a current-role question.
Is the Job Interest Form template free to use?
Yes. You can preview and test Job Interest Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.
What does the Job Interest Form template ask for?
It asks 11 questions across 1 page, 6 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.
Can I edit the Job Interest Form form after copying it?
Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the card and classic layouts, restyle it to match your brand, and set who gets notified on each submission.
How does the conditional logic in this human resources form work?
3 conditional rules ship with the template: Features advanced show logic: if “Is it OK to keep your details on file?” is No, the form dynamically exposes “What happens to your details”. Otherwise that question never appears. Features advanced show logic: if “Employment type” contains Seasonal, the form dynamically exposes “Which season?”. Otherwise that question never appears. Features conditional validation: if “Employment type” contains Seasonal, “Which season?” becomes mandatory before the form can be sent. All rules are editable in the Logic tab.
Where do responses to the Job Interest Form form go?
Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.
Can I embed the Job Interest Form form on my own website?
Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.
Can respondents upload files?
Yes. This template includes a file upload question, so respondents can attach documents or photos with their answers. You can adjust the accepted file types and size limits on that question.
