Any organisation that works with under-18s needs a reusable consent form, and this one is written to be adapted rather than rewritten each time. The parent or guardian identifies themselves, confirms they hold parental responsibility, and names the child with their date of birth. The activity or purpose being consented to is described specifically, with its date or date range, location and the organisation responsible, so consent is never vague or open-ended. Consent items are listed individually — participation, transport, photography, off-site activity, emergency medical treatment, over-the-counter medication and internet or online platform use — because a single blanket tick box is exactly what a safeguarding review will question. Medical needs, allergies, medication and a doctor's surgery are recorded along with two emergency contacts reachable during the activity. The parent confirms they have read any risk information provided and understand they may withdraw consent in writing at any time. The signature and timestamp complete a record that stands up to scrutiny and is easy to reissue each term.
