About this template
Salons lose money on badly estimated appointment lengths, and this booking form is built to prevent that by asking the questions a stylist would ask on the phone. The client gives their name, mobile number and email, says whether they have visited before, and picks the services they want from a list you control — cut, blow dry, colour, highlights, balayage, toner, treatment or extensions. Colour work triggers extra questions about current colour, box dye history, previous bleaching and whether a patch test has been done, which is both a timing and a safety issue. Hair length and thickness refine the slot length further. The client then chooses a preferred stylist, a date and a time window, and can note anything relevant such as a wedding deadline or a scalp sensitivity. A short consent block covers the patch-test requirement, deposit terms and your cancellation window. Every submission arrives with enough detail to be dropped into the diary at the right length, first time.
