A short structured form fixes that without asking users for more than a minute. Satisfaction and likelihood to recommend are captured as separate numbers because they answer different questions: one is about today, the other about the relationship. Detractors are asked directly what would improve their score, which produces the most actionable text on the form. The most-used feature question tells you which parts of the product justify their maintenance cost, and the frustration question is where roadmap items are actually born. Role and usage frequency turn the whole dataset into segments, so a complaint from a daily administrator is not weighted the same as one from a monthly viewer. Consent for follow-up requires an email, keeping the rest of the form genuinely anonymous.
