Who this template is for

Product-Market Fit Survey is built for product, support and customer-experience teams who need to measure satisfaction and capture the reason behind every score.

  • Product, support and customer-experience teams working in surveys & feedback.
  • Teams who need to measure satisfaction and capture the reason behind every score without writing code or paying for a custom build.
  • Anyone replacing ratings with no context, so nothing actionable comes out of them with one structured record per enquiry.
  • Respondents are customers, users and members — the form asks them 5 questions across 1 screen.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of ratings with no context, so nothing actionable comes out of them and turns each submission into a record your team can act on immediately.

  • 1 question is required, so submissions arrive complete instead of needing a follow-up email.
  • Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

About this template

This is the widely used product-market-fit test built around a single, deceptively simple question: how disappointed would the respondent be if they could no longer use the product. That one question, asked of active users, predicts retention and growth far better than a satisfaction score does, because it forces a real trade-off rather than polite praise. Respondents who say they'd be very disappointed are the strongest signal of fit, so the survey branches to ask them two follow-ups: what specific benefit they'd miss most, described in their own words, and what type of person they think would benefit most from the product — a rough proxy for their own persona that's often more honest than a direct demographic question. Everyone else — somewhat disappointed or not disappointed — branches instead into an improvement question asking what the product would need to do better to earn a higher answer, which is the most direct source of prioritized feedback a product team can get. The survey closes with an optional main-benefit question for everyone regardless of branch, since even lukewarm users can usually name the one thing keeping them around. Kept to a single page and under five questions per respondent, it's meant to be run on a recurring cadence against active users, not as a one-off.

Driven by “How would you feel if you could no longer use this product?

  • Reveals questions

If “How would you feel if you could no longer use this product?” is Somewhat disappointed, the form reveals “What would make this a lot better for you?”.

  • Reveals questions

If “How would you feel if you could no longer use this product?” is Not disappointed, the form reveals “What would make this a lot better for you?”.

Set it up step by step

  1. 1

    Only survey active users

Send this to people who've actually used the product recently — asking inactive or trial-only users skews the score badly.

  2. 2

Treat 40% as the benchmark, not the target

A common rule of thumb is that 40%+ answering 'very disappointed' signals fit; track the trend over time rather than chasing one snapshot.

  3. 3

Mine the persona answer for positioning

The 'what type of person' answer from very-disappointed users is often better marketing copy than anything written internally.

  4. 4

Prioritize the improvement answers by segment

Weight 'what would make this better' feedback from your most engaged users higher than from users who barely use the product.

Mistakes to avoid

  • Surveying trial users or people who haven't yet gotten value, which understates true product-market fit.
  • Skipping the branch questions and only tracking the score, missing the qualitative detail that explains it.
  • Running it once and never again, when the real value comes from tracking the percentage over time as the product changes.

Frequently asked questions

Who should receive this survey?

Active users who've used the core feature at least a couple of times — new signups and dormant accounts will skew results downward.

What response rate or score is good?

Around 40% answering 'very disappointed' is a commonly cited threshold for strong fit, but trend direction over repeated surveys matters more than any single number.

How is this different from a satisfaction survey?

Satisfaction scores measure how users feel about the experience; this measures how essential the product actually is to them, which better predicts retention.

How often should I run this?

Quarterly is typical for an established product, though early-stage teams often run it monthly while iterating quickly.
