Page two covers the return itself — fit for full duties, with adjustments, or a phased return — plus the adjustments agreed, an optional doctor's note upload, confirmation that the manager interview took place, and the employee's signature. Pairing the compliance record with the interview step is the point: the signature is what turns a conversation into evidence that a duty of care was met. It differs from the Leave Request Form, which is completed before an absence to ask permission; this one is completed afterwards to close it.
