Who this template is for
Vehicle Inspection Form is built for garages, dealers and fleet operators who need to capture vehicle detail, service needs and contact preferences.
- Garages, dealers and fleet operators working in automotive, travel & more.
- Teams who need to capture vehicle detail, service needs and contact preferences without writing code or paying for a custom build.
- Anyone replacing quotes based on missing vehicle information with one structured record per enquiry.
- Respondents are vehicle owners and buyers — the form asks them 27 questions across 4 screens.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of quotes based on missing vehicle information and turns each submission into a record your team can act on immediately.
- 18 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It accepts a document or photo upload as evidence.
- It records a signature you can keep as proof of agreement.
- It pins the request to a specific date.
- It captures a numeric value you can filter and sort on.
- Conditional logic hides 5 questions until they are relevant — shorter forms convert better than long ones.
- Splitting the form across 4 pages keeps each screen short and shows respondents how much is left.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 5 conditional rules ship with it.
- Features advanced show logic: if “Were any defects found?” is Yes, the form dynamically exposes “Describe each defect”. Otherwise that question never appears.
- Features conditional validation: if “Were any defects found?” is Yes, “Defect severity” becomes mandatory before the form can be sent.
- Features conditional validation: if “Is the vehicle roadworthy?” is No, “Who has been notified” becomes mandatory before the form can be sent.
- Features advanced show logic: if “Is the vehicle roadworthy?” is No, the form dynamically exposes “Vehicle taken out of service at”. Otherwise that question never appears.
- Features conditional validation: if “Type of check” is Handover, “Driver taking over” becomes mandatory before the form can be sent.
- All 5 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
A daily vehicle check only protects you if the defect it found can be traced to a driver, a time and a decision. This form produces that trail. The driver identifies the vehicle by registration and fleet number, records the odometer reading and states whether this is a start-of-shift, end-of-shift or handover check. The walk-around runs through the items an enforcement officer looks at first: tyres and pressures, lights and indicators, mirrors and glass, brakes, fluid levels, bodywork, load security and the presence of safety equipment. Each item is graded pass, defect or not applicable, and any defect opens a block for the description, severity and photographs taken from the vehicle rather than remembered later. The critical question follows: is the vehicle roadworthy. Answering no requires the name of the person notified and takes the vehicle out of service in your records, which is the step that gets skipped on paper. Fuel or charge level and any adblue or fluid top-ups round out the entry, and the driver signs to own the check.

How the form changes as it's filled in
This template ships with 5 conditional rules, grouped into 5 behaviours driven by 3 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

- Reveals questions

Set it up step by step
- 1
Match the checklist to your fleet
Swap the walk-around rows for the items on your own defect card — vans, HGVs and plant each need a different list.
- 2
Run it in card mode
One check per screen suits a driver walking round a vehicle in the rain far better than a long scrolling page.
- 3
Alert the workshop on failure
Route a notification when roadworthy is answered No so the vehicle is grounded before the next shift picks up the keys.
- 4
Keep the records
Retain completed checks for the period your operator licence or insurer requires and export them by registration when audited.
Mistakes to avoid
- Ticking every box pass out of habit without a photo requirement for defects.
- Recording a defect but leaving the vehicle in service with nobody notified.
- Not capturing the odometer, which makes wear-related defects impossible to trend.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Ground any vehicle marked not roadworthy, raise a repair job from the defect description, and re-check before it returns to service.

Frequently asked questions
Does this replace a statutory daily check?
It records one. The legal requirement is set by your operator licence or local regulator, so align the checklist rows with the defect list they publish.
Can drivers complete it without signal?
Yes. Load the form at the depot; answers are held in the browser and submit when the device reconnects.
How do I stop everything being marked pass?
Make defect photographs required through the logic rule, and review the ratio of clean checks per driver — a driver who never finds anything is worth a conversation.
Can I use it for vehicle handovers?
Choose Handover as the check type and the incoming driver's name becomes required, giving you a clean condition record between the two people.
Is the Vehicle Inspection Form template free to use?
Yes. You can preview and test Vehicle Inspection Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.
What does the Vehicle Inspection Form template ask for?
It asks 27 questions across 4 pages, 18 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.
Can I edit the Vehicle Inspection Form form after copying it?
Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.
Can respondents upload files?
Yes. This template includes a file upload question, so respondents can attach documents or photos with their answers. You can adjust the accepted file types and size limits on that question.
Is the signature on this form legally usable?
The template captures a drawn signature and stores it with the submission, along with the timestamp, so you keep evidence of who agreed and when. Whether that is sufficient depends on your jurisdiction and the agreement itself.
