Remote access granted for one project and never removed is how a small supplier becomes a permanent open door. This form makes access temporary by default. The first page covers the person: name, whether they are staff, a contractor or a supplier, the company they work for, their manager or sponsor inside the business, and what they need to do that requires reaching internal systems, described as a task. It asks which systems or applications they need, whether they need to change things or only look, and the countries they will connect from, since that answer often decides whether the request is approved at all. The second page covers the device and the duration: whether they will use a company or personal machine, whether it is encrypted and running current updates, whether anyone else uses that machine, the start and end dates, expected hours of use and whether access is needed outside working hours. A short set of confirmations records the conditions the person accepts. Your own security standards decide what is granted, so the form gathers facts and an approval trail.
