Who this template is for

Business Bank Account Application Form is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in banking & identity.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 32 questions across 5 screens.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 21 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It accepts a document or photo upload as evidence.
  • It records a signature you can keep as proof of agreement.
  • It pins the request to a specific date.
  • It captures a full postal address.
  • It captures a numeric value you can filter and sort on.
  • Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 4 conditional rules ship with it.

  • Features advanced show logic: if “Does the business trade outside its home country?” is Yes, the form dynamically exposes “Which countries do you trade with?”. Otherwise that question never appears.
  • Features advanced show logic: if “Is any owner a company, trust or partnership?” is Yes, the form dynamically exposes “Describe the ownership structure above the company”. Otherwise that question never appears.
  • Features conditional validation: if “Do payments need more than one approver?” is Yes, “How many approvers are required?” becomes mandatory before the form can be sent.
  • Features advanced show logic: if “Are you moving an existing business account?” is Yes, the form dynamically exposes “Current bank and account details”. Otherwise that question never appears.
  • All 4 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

A business account application stalls on the same three things every time: who owns the company, what it actually does, and who is allowed to move money. This form asks all three properly. The company block records the registered name, trading name, registration number, incorporation date, registered and trading addresses, legal structure and website. The activity block asks what the business sells, to whom, in which countries, and the expected monthly turnover in and out — with the countries question expanding when trade crosses borders, because that is what drives enhanced checks. Ownership is captured as a list of everyone holding a meaningful stake, each with their name, date of birth, address, percentage held and identity document, and a separate answer flags where ownership sits behind a trust or holding company. Signatories are recorded with their role, the level of authority they hold and whether payments need dual approval. The banking section covers the product wanted, currency, card requirements and any existing accounts being moved across. A director's declaration, signature and date complete the record so the whole file arrives at review in one piece rather than in six emails.
