This form closes that gap without making the customer fill in a mortgage application. The company block captures the registered name, trading name, registration number, legal structure, incorporation date and both registered and delivery addresses. The request itself is specific: the credit limit wanted, the payment terms sought, the expected monthly spend, and what the account will be used for — four answers that let a credit controller sanity-check the limit against likely usage before running any external check. Trade references are collected as structured entries with company name, contact, email and how long the relationship has run, and a bank reference block appears where your policy requires one. Financial questions ask for annual turnover, whether audited accounts are available, and whether the business has had a county court judgment, default or insolvency event, with an explanation block that only appears when the answer calls for it. A personal guarantee section is included and can be switched on for smaller or newer businesses. The authorised signatory signs the terms and the completed application exports as one file for the credit file.
