The first page identifies the parties and the job: project name and job number, site address, the package or trade, the company giving the waiver with its registration number, and who it is given to. It records the payment application or invoice number, the period the work covers and the amount, along with any retention held and any approved change orders included, since a waiver that ignores a change order is where disputes begin. The second page sets the scope: partial waiver for this payment only, or final waiver on completion, plus whether anything is expressly excluded such as pending change orders or unresolved claims, confirmation that suppliers and lower-tier trades for the period have been paid, and a signature with date and capacity.
