Who this template is for

Subcontractor Bid Form is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in construction.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 35 questions across 3 screens.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 25 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It captures a phone number for urgent follow-up.
  • It accepts a document or photo upload as evidence.
  • It records a signature you can keep as proof of agreement.
  • It captures a numeric value you can filter and sort on.
  • Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 3 pages keeps each screen short and shows respondents how much is left.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 4 conditional rules ship with it.

  • Features conditional validation: if “Will you supply materials for this package?” is Yes, “Materials included in the price” becomes mandatory before the form can be sent.
  • Features conditional validation: if “Do you need plant provided by the main contractor?” is Yes, “Which plant you need provided” becomes mandatory before the form can be sent.
  • Features conditional validation: if “Are RAMS ready for this package?” is No, we would write them after award, “When RAMS would be available” becomes mandatory before the form can be sent.
  • Features conditional validation: if “Any part of this package sublet?” is Yes, “What is sublet, and to whom” becomes mandatory before the form can be sent.
  • All 4 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

Choosing a trade on price alone is how a site ends up with two operatives where six were promised. This three-page form asks for the price and the resource behind it. The first page identifies the package: project and job number, package title, the phase and area it covers, the drawing revision priced, and the bidding trade with its registration, insurance and any accreditation. The second page prices the work — rate structure, labour, materials if supplied, plant, the total, and any day rates for extra work, so variations do not need a fresh negotiation later. The third page covers delivery, and it is the page that separates bids: operatives on site each week, who supervises them, how many hold the required tickets, whether method statements and risk assessments will be provided before starting, waste responsibility, and lead time from instruction. Because insurance and safety documents are asked for at bid stage, the winner does not spend a week chasing paperwork before the first shift. Appointment terms and retention are agreed in the subcontract, not here.

How the form changes as it's filled in

This template ships with 4 conditional rules, grouped into 4 behaviours driven by 4 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Will you supply materials for this package?

  • Makes answers required

If “Will you supply materials for this package?” is Yes, “Materials included in the price” becomes required before the form can be submitted.

Driven by “Do you need plant provided by the main contractor?

  • Makes answers required

If “Do you need plant provided by the main contractor?” is Yes, “Which plant you need provided” becomes required before the form can be submitted.

Driven by “Are RAMS ready for this package?

  • Makes answers required

If “Are RAMS ready for this package?” is No, we would write them after award, “When RAMS would be available” becomes required before the form can be submitted.

Driven by “Any part of this package sublet?

  • Makes answers required

If “Any part of this package sublet?” is Yes, “What is sublet, and to whom” becomes required before the form can be submitted.

Set it up step by step

  1. 1

Ask every trade the same questions

Send the same link to all bidders for a package so labour numbers and exclusions sit side by side rather than in five different formats.

  2. 2

    Read page three first

Operatives, supervision and tickets predict whether a package finishes on time far better than the total price does.

  3. 3

    Collect certificates at bid stage

The attachment field means the winner's insurance and RAMS are already on file before the first shift.

  4. 4

    Bank the day rate

Having an agreed day rate in the bid means small extras become a change order rather than a negotiation.

Mistakes to avoid

  • Awarding on total price without comparing crew size, then discovering the programme was never achievable.
  • Chasing insurance and RAMS after award, which loses the first week of the package.
  • Leaving waste responsibility unstated, so skips and clearing appear as an unbudgeted cost.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Shortlist on crew and exclusions as well as price, save the winner's certificates against the job number, and reuse the agreed day rate in every change order on that package.

Works in this layout

These pages cover the same subject ground as the subcontractor bid form, matched on the words people actually search for.

Privacy & compliance

This form takes money. Card details are handled by the payment provider and never stored in your response data — you keep the order details, not the card number.

Frequently asked questions

How is this different from a subcontractor agreement?

The agreement is the appointment itself, with terms and signatures. This is the bid before it: what the trade would charge and what resource sits behind the number.

Is it only for main contractors?

Mostly. A homeowner comparing two trades is better served by a quote request form; this assumes a job number, packages and site supervision.

Should we ask for rates or a lump sum?

Ask which they are offering. The rate structure question means measured-rate and fixed-price bids stay comparable instead of being flattened into one number.

Can we use the answers as a contract?

No. Treat them as the priced offer and put the appointment in a subcontract; the form deliberately stops short of terms.

Is the Subcontractor Bid Form template free to use?

Yes. You can preview and test Subcontractor Bid Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

What does the Subcontractor Bid Form template ask for?

It asks 35 questions across 3 pages, 25 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.

Can I edit the Subcontractor Bid Form form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic layout, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this construction form work?

4 conditional rules ship with the template: Features conditional validation: if “Will you supply materials for this package?” is Yes, “Materials included in the price” becomes mandatory before the form can be sent. Features conditional validation: if “Do you need plant provided by the main contractor?” is Yes, “Which plant you need provided” becomes mandatory before the form can be sent. Features conditional validation: if “Are RAMS ready for this package?” is No, we would write them after award, “When RAMS would be available” becomes mandatory before the form can be sent. All rules are editable in the Logic tab.

Where do responses to the Subcontractor Bid Form form go?

Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.

Can I embed the Subcontractor Bid Form form on my own website?

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.

Can respondents upload files?

Yes. This template includes a file upload question, so respondents can attach documents or photos with their answers. You can adjust the accepted file types and size limits on that question.

Is the signature on this form legally usable?

The template captures a drawn signature and stores it with the submission, along with the timestamp, so you keep evidence of who agreed and when. Whether that is sufficient depends on your jurisdiction and the agreement itself.
