Who this template is for

Stock Alert Signup Card is built for teams who want a conversational, one-question-at-a-time flow who need to keep completion high by asking one thing per screen.

  • Teams who want a conversational, one-question-at-a-time flow working in card forms.
  • Teams who need to keep completion high by asking one thing per screen without writing code or paying for a custom build.
  • Anyone replacing long scrolling forms abandoned halfway with one structured record per enquiry.
  • Respondents are anyone answering on a phone — the form asks them 9 questions across 1 screen.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of long scrolling forms abandoned halfway and turns each submission into a record your team can act on immediately.

  • 6 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It captures a phone number for urgent follow-up.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “Variant” is Other variant, the form dynamically exposes “Other Variant Details”. Otherwise that question never appears.
  • Features advanced show logic: if “Alert Delivery” is Email and text message, the form dynamically exposes “Mobile Number”. Otherwise that question never appears.
  • Features advanced show logic: if “Shopping Location” is Store pickup, the form dynamically exposes “Preferred Store”. Otherwise that question never appears.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

An unavailable product should not send shoppers into a dead end. This card gives them a quick way to request an alert for a particular item, variant, and shopping location. It collects an email address by default and offers an optional text message without turning a restock request into a general newsletter signup. Place the card beside the unavailable purchase button. Prefill the product name or code from that page, and replace the sample variant choices with the actual options for the item. Keep product identifiers consistent with your inventory system; a free-text product nickname is difficult to match when stock arrives. If you offer store pickup, customize the location prompt so shoppers can identify the branch they mean. Explain before submission that an alert does not reserve an item or guarantee availability. Do not ask for payment details, shipping addresses, or an account password. Those belong in a purchase flow, not an availability request. Route requests into a product-and-variant queue, then check sellable inventory before sending a notification. Include a direct product link and a way to stop future alerts. Deduplicate repeated requests for the same email, item, variant, and location. After sending an alert, mark that request as notified so the next inventory update does not trigger another unwanted message.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 3 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Variant

  • Reveals questions

If “Variant” is Other variant, the form reveals “Other Variant Details”.

Driven by “Alert Delivery

  • Reveals questions

If “Alert Delivery” is Email and text message, the form reveals “Mobile Number”.

Driven by “Shopping Location

  • Reveals questions

If “Shopping Location” is Store pickup, the form reveals “Preferred Store”.

Set it up step by step

  1. 1

Tie the card to one product

Embed it on the unavailable product page and prefill Product Name or Code with a stable inventory identifier. Replace the sample variant menu before publishing.

  2. 2

Offer only channels you can deliver

Keep email as the baseline. Remove the text-message option, Mobile Number field, and related rule if your team has no SMS workflow.

  3. 3

    Match the right inventory pool

Separate online requests from store-pickup requests. Confirm missing store or variant details before matching a request to available stock.

  4. 4

    Close the loop after notification

Check that the item can actually be purchased, send its direct link, and record the notification date. Suppress duplicate requests and stop alerts when a shopper opts out.

Mistakes to avoid

  • Leaving generic size choices on products that need color, model, or combined variant selections.
  • Treating a stock alert signup as a preorder, stock reservation, or newsletter subscription.
  • Sending alerts when inventory is received but still unavailable for purchase or pickup.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Show a confirmation naming the requested product and explaining that no stock is reserved. Route the request to the matching inventory queue and notify the shopper only after availability is verified.

Works in both layouts

These pages cover the same subject ground as the stock alert signup card, matched on the words people actually search for.

Frequently asked questions

Does signing up reserve a product?

No. This card records interest and notification preferences only. Use the Product Pre-Order Card if you are accepting advance orders through a separate purchase process.

Should shoppers enter a product name themselves?

Prefer a prefilled product name and stable code from the product page. Keep manual entry for a standalone card, and review unmatched entries before adding them to an automated alert queue.

What happens if someone requests texts but leaves the number blank?

Send the alert by email only. The mobile field is optional, so your notification workflow should check that a usable number exists before attempting a text message.

Can this card send alerts automatically?

The card collects requests; automatic delivery needs a connected inventory and messaging workflow. Match product, variant, and location, verify sellable stock, and mark each request as notified after sending.

Is the Stock Alert Signup Card template free to use?

Yes. You can preview and test Stock Alert Signup Card on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this card forms form work?

3 conditional rules ship with the template: Features advanced show logic: if “Variant” is Other variant, the form dynamically exposes “Other Variant Details”. Otherwise that question never appears. Features advanced show logic: if “Alert Delivery” is Email and text message, the form dynamically exposes “Mobile Number”. Otherwise that question never appears.
