  • Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
  • Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
  • Legal text not required — passingThis template collects neither health nor regulated financial information.
  • Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.

Who this template is for

Warranty Registration Card is built for teams who want a conversational, one-question-at-a-time flow who need to keep completion high by asking one thing per screen.

  • Teams who want a conversational, one-question-at-a-time flow working in card forms.
  • Teams who need to keep completion high by asking one thing per screen without writing code or paying for a custom build.
  • Anyone replacing long scrolling forms abandoned halfway with one structured record per enquiry.
  • Respondents are anyone answering on a phone — the form asks them 10 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of long scrolling forms abandoned halfway and turns each submission into a record your team can act on immediately.

  • 5 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It accepts a document or photo upload as evidence.
  • It pins the request to a specific date.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “Purchase Channel” is Retail Store, the form dynamically exposes “Retailer Name”. Otherwise that question never appears.
  • Features advanced show logic: if “Purchase Channel” is Manufacturer's Online Store, the form dynamically exposes “Order Number”. Otherwise that question never appears.
  • Features advanced show logic: if “Receipt Available” is Yes, the form dynamically exposes “Proof of Purchase”. Otherwise that question never appears.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

A warranty registration card should create a usable product record without turning into a support ticket. Collect the customer's name and email, the product model, and the purchase date first. Keep serial numbers as text so leading zeros and hyphens survive. Add a short location hint based on your actual packaging or product label; customers should not need to hunt through a manual to finish. Use the purchase channel to reveal either a retailer name or an order number. Gifts need a separate path so recipients are not forced to invent a seller or order reference. Offer receipt upload only when the customer has a copy, and accept a clear photo as well as a PDF. Do not collect a mailing address unless your registration process actually uses it. Before publishing, replace the generic product hint with your model naming convention and explain how submitted details will be used. Keep promotional subscriptions out of the registration flow. Send a confirmation containing the registered model and a reference number, but do not describe submission as warranty approval. Route missing or conflicting purchase details to a review queue rather than silently rejecting the record.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Purchase Channel

  • Reveals questions

If “Purchase Channel” is Retail Store, the form reveals “Retailer Name”.

  • Reveals questions

If “Purchase Channel” is Manufacturer's Online Store, the form reveals “Order Number”.

Driven by “Receipt Available

  • Reveals questions

If “Receipt Available” is Yes, the form reveals “Proof of Purchase”.

Set it up step by step

  1. 1

Make model codes easy to find

Replace the example model with one from your catalog and specify where its label sits. Keep model and serial number fields as text to preserve leading zeros.

  2. 2

    Test each purchase path

Submit one test for each purchase channel. Confirm that Retailer Name appears only for Retail Store and Order Number only for Manufacturer's Online Store.

  3. 3

    Set receipt upload limits

Configure uploads to accept JPG, PNG, and PDF files with a size limit suitable for phone photos. Test a photographed receipt on mobile before sharing the card.

  4. 4

    Separate records from decisions

Assign each submission a registration reference and send an acknowledgment. Flag missing dates or duplicate serial numbers for review without treating registration as an approved claim.

Mistakes to avoid

  • Requiring a receipt, purchase date, or order number from every gift recipient, which encourages guessed information.
  • Using a numeric serial number field that removes leading zeros or rejects letters and hyphens.
  • Sending a confirmation that promises coverage instead of simply confirming receipt of the registration.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Send the customer a registration reference and a copy of the product details they submitted. Store the record with your product support data and route incomplete or duplicate entries for review.

Works in both layouts

These pages cover the same subject ground as the warranty registration card, matched on the words people actually search for.

Frequently asked questions

Should proof of purchase be mandatory?

This template keeps the upload optional so customers can register without a receipt immediately available. If your process needs a copy later, explain that follow-up in the confirmation rather than blocking the initial record.

Can customers register a product they received as a gift?

Yes. The Gift option bypasses seller-specific fields, and the purchase date can stay blank. Ask for missing information later only if your team needs it.

Does this card also handle repairs or warranty claims?

No. It creates a registration record, not a repair request or a coverage decision. Link to the Say Hello Contact Card for general questions, and direct repair requests to your designated support process.

How should duplicate serial numbers be handled?

Flag them for review rather than automatically deleting or rejecting the newer submission. A duplicate may be a correction, a mistyped code, or a product registered by another household member.

Is the Warranty Registration Card template free to use?

Yes. You can preview and test Warranty Registration Card on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

What does the Warranty Registration Card template ask for?

It asks 10 questions across 2 pages, 5 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.

Can I edit the Warranty Registration Card form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this card forms form work?

3 conditional rules ship with the template: Features advanced show logic: if “Purchase Channel” is Retail Store, the form dynamically exposes “Retailer Name”. Otherwise that question never appears. Features advanced show logic: if “Purchase Channel” is Manufacturer's Online Store, the form dynamically exposes “Order Number”. Otherwise that question never appears. Features advanced show logic: if “Receipt Available” is Yes, the form dynamically exposes “Proof of Purchase”. Otherwise that question never appears. All rules are editable in the Logic tab.

Where do responses to the Warranty Registration Card form go?

Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.

Can I embed the Warranty Registration Card form on my own website?

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.

Can respondents upload files?

Yes. This template includes a file upload question, so respondents can attach documents or photos with their answers. You can adjust the accepted file types and size limits on that question.
