It records the notice date and the last working day being proposed, then asks whether the employee wants to work the notice, be paid in lieu, or use remaining holiday against it — the three options that otherwise get settled in an awkward second conversation. It asks for the reason as an option set with a free-text field, and says plainly that the answer is voluntary, because a resignation is not the place to insist on candour. The rest is logistics that would otherwise be chased for two weeks: who should take on which responsibilities, what equipment and access needs returning, and the personal email for the final payslip and any documents.
