About this template

Junior clubs need a complete record for every player before they set foot on a pitch, and volunteer coaches rarely have time to chase it. This registration form collects the player's full name, date of birth, school year and preferred position, then the age group or team they are joining. Parent and guardian details are captured with a second emergency contact, because the first one is always the one who is coaching. The medical section records conditions such as asthma, epilepsy, diabetes and allergies, current medication, whether the player carries an inhaler or EpiPen, and consent for a first aider to administer emergency treatment. Photography and social media consent is asked separately for children, as safeguarding policy requires. Kit sizes, subscription payment preference and availability for training nights complete the record. A code of conduct for players and spectators is accepted by the guardian, and the whole thing is signed on screen, giving the club a dated, auditable registration for the season.
