About this template

Transcript requests arrive by email with half the details missing, and the registrar spends a week asking follow-up questions instead of sending records. One screen fixes that. It asks for the student's full name, any former name the records might be filed under, date of birth, student number if it is known, and the years attended, because a request without a date range means searching an archive by hand. It records the qualification or programme, how many copies are wanted, and the format: sealed paper copy, electronic copy, or sent directly to a third party. When the destination is a third party the form asks for the recipient's name and address, since that is where most requests stall.
