Compliance checklist
- Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
- Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
- Financial disclaimer present — passingThis form does not provide financial, tax or legal advice. Any figures shown are indicative only and are not an offer of credit or a guarantee of terms. Please seek advice from a regulated adviser before making a decision.
- Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.
What happens after submit: nothing is stored in this preview. On your own copy, every response triggers an instant email notification and lands in your response dashboard, ready to export or forward.

Who this template is for
Exam Registration Form is built for schools, tutors and course providers who need to enrol people and record permissions in one place.
- Schools, tutors and course providers working in education & schools.
- Teams who need to enrol people and record permissions in one place without writing code or paying for a custom build.
- Anyone replacing consent forms chased on paper the week before term with one structured record per enquiry.
- Respondents are students, parents and staff — the form asks them 13 questions across 2 screens.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of consent forms chased on paper the week before term and turns each submission into a record your team can act on immediately.
- 6 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It accepts a document or photo upload as evidence.
- It pins the request to a specific date.
- Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.
- Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 4 conditional rules ship with it.
- Features advanced show logic: if “Access arrangements needed?” is Yes, the form dynamically exposes “Access arrangement details”. Otherwise that question never appears.
- Features conditional validation: if “Access arrangements needed?” is Yes, “Access arrangement details” becomes mandatory before the form can be sent.
- Features advanced show logic: if “Fee payment method” is Invoice, the form dynamically exposes “Billing contact”. Otherwise that question never appears.
- Features conditional validation: if “Fee payment method” is Invoice, “Billing contact” becomes mandatory before the form can be sent.
- All 4 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
Exam entries fail for administrative reasons far more often than academic ones: a name that does not match the passport, an access arrangement raised too late, a fee nobody invoiced. This form closes all three gaps. Page one identifies the candidate — full name as it appears on identification, date of birth, email, phone and candidate or student ID. Page two handles the sitting: subject, level, date, whether access arrangements are needed, and how the fee is being paid. It differs from the Course Enrolment & Exam Registration Form, which enrols someone onto a course of study that happens to end in an assessment. This one registers a candidate for a single sitting, including external candidates who never took your course. Two rules protect the deadline-critical bits: answering yes to access arrangements reveals a required details field and an evidence upload, so nothing is approved on an assertion alone, and choosing invoice payment reveals a required billing contact, which is the difference between a fee paid and a fee chased.

How the form changes as it's filled in
This template ships with 4 conditional rules, grouped into 4 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “Access arrangements needed?”
- Reveals questions
If “Access arrangements needed?” is Yes, the form reveals “Access arrangement details”.
- Makes answers required
If “Access arrangements needed?” is Yes, “Access arrangement details” becomes required before the form can be submitted.
Driven by “Fee payment method”
- Reveals questions
If “Fee payment method” is Invoice, the form reveals “Billing contact”.
- Makes answers required
If “Fee payment method” is Invoice, “Billing contact” becomes required before the form can be submitted.
Set it up step by step
- 1
Ask for the identity-document name
Say clearly that the name must match the identification brought on the day; it prevents the most common exam-day refusal.
- 2
Load your real sittings
Replace the placeholder dates with the sittings you are actually running, and remove them as they fill.
- 3
Set your own deadline first
Close entries a week before the awarding body deadline so access arrangements can be processed properly.
- 4
Confirm in writing
Send each candidate the date, venue, arrival time and identification requirements as soon as the entry is approved.
Mistakes to avoid
- Asking about access arrangements without asking for evidence, which leaves the entry unapprovable close to the deadline.
- Recording a name that does not match the candidate's identity document, which invalidates the entry on exam day.
- Offering invoicing without collecting a billing contact, so the fee is chased instead of paid.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Check each entry against the awarding body deadline, approve access arrangements with the evidence attached, raise invoices from the billing contacts, and send every candidate their sitting date, venue and identification requirements.
Works in both layouts
