  • Financial disclaimer present — passingThis form does not provide financial, tax or legal advice. Any figures shown are indicative only and are not an offer of credit or a guarantee of terms. Please seek advice from a regulated adviser before making a decision.

Who this template is for

Course Enrollment Form is built for schools, tutors and course providers who need to enrol people and record permissions in one place.

  • Schools, tutors and course providers working in education & schools.
  • Teams who need to enrol people and record permissions in one place without writing code or paying for a custom build.
  • Anyone replacing consent forms chased on paper the week before term with one structured record per enquiry.
  • Respondents are students, parents and staff — the form asks them 14 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of consent forms chased on paper the week before term and turns each submission into a record your team can act on immediately.

  • 7 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It captures a phone number for urgent follow-up.
  • It pins the request to a specific date.
  • It captures a full postal address.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “How will you pay?” is Funding body, the form dynamically exposes “Funding body name”. Otherwise that question never appears.
  • Features conditional validation: if “How will you pay?” is Funding body, “Funding body name” becomes mandatory before the form can be sent.
  • Features advanced show logic: if “Prior experience” is Advanced, the form dynamically exposes “Request a placement test”. Otherwise that question never appears.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

Enrolment is where a course provider collects the only administrative data it will have for the rest of the programme, so the form has to be complete without feeling like an application. The first page is pure student record: contact details, date of birth, address and an emergency contact that safeguarding policies almost always require and enrolment forms almost always forget. The second page is the commercial half — which course, which start date, what the student already knows, and how they intend to pay. Payment preference is a three-way choice because invoice, card link and third-party funding create genuinely different back-office work; choosing funding requires the funding body immediately, which is the detail that otherwise takes three emails to extract. Declaring advanced prior experience offers a placement test rather than assuming it, which quietly prevents the most common first-week withdrawal. A terms agreement closes the form with a timestamped record.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “How will you pay?

  • Reveals questions

If “How will you pay?” is Funding body, the form reveals “Funding body name”.

  • Makes answers required

If “How will you pay?” is Funding body, “Funding body name” becomes required before the form can be submitted.

Driven by “Prior experience

  • Reveals questions

If “Prior experience” is Advanced, the form reveals “Request a placement test”.

Set it up step by step

  1. 1

Match the course list to your prospectus

Course and start date should read exactly as they do on the page the student enrolled from, or reconciliation becomes manual.

  2. 2

    Send the payment route immediately

Filter by payment preference and send an invoice, a card link or a funding pack the same day.

  3. 3

Check the emergency contact is complete

It is the field most often left blank and the one you will need at the worst possible moment.

  4. 4

    Act on placement test requests

A ten-minute test before week one saves a withdrawal and a refund in week three.

Mistakes to avoid

  • Taking payment details inside the enrolment form rather than sending a proper payment link or invoice.
  • Offering a free-text start date, which produces intakes that cannot be counted.
  • Skipping the terms agreement, leaving cancellation and refund policy unacknowledged.

These pages cover the same subject ground as the course enrollment form, matched on the words people actually search for.

Frequently asked questions

Can I take payment on the form itself?

You can add a priced product question, but most providers prefer sending an invoice or card link after checking eligibility.

Is it suitable for under-18 students?

Add a parent or guardian section and a consent question; the emergency contact fields already cover part of what you will need.

How do I handle waiting lists?

Add a "waiting list" option to the start date dropdown and filter on it rather than running a second form.

What should follow enrolment?

The Course Evaluation Form at the end of the programme, so intake and outcome sit in the same reporting.
