About this template

Anonymity fails when a form promises it in the intro and then asks for a name in the middle. This template has no name field, no email field and no required identifier anywhere — the only contact question sits last, is clearly marked optional, and exists solely so somebody who wants a reply can ask for one. The questions are built to make vague feedback harder: which area it concerns, what kind of feedback it is, the substance itself, how serious the person considers it, how long it has been going on, and what fixing it would look like. That last question turns a complaint into a proposal. It differs from the Employee Engagement Survey, which measures sentiment across a whole workforce on a schedule — this one is an always-open channel for a single issue. Two rules add depth without adding fields: an urgent issue report shows an escalation note explaining what happens next, and answering that the concern was raised before opens a question about what happened then.

Is it really anonymous?

There is no identifying question in the template, and the optional contact field is the last one on the form. Turn off IP capture in the builder's response settings to remove the last trace.

Why is the contact field there at all?

Some people want a reply, and forcing them into a second channel loses the thread. It is optional, unlabelled as a name, and placed last so nobody feels obliged.

Can I still filter by team?

Yes — the area question gives you grouping without identity. Keep the list broad enough that no option maps to a single person.

What if someone reports something serious?

The urgency question exists for exactly that. Agree in advance who reads urgent submissions and how quickly, and say so in the escalation note.
