  • Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
  • Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
  • Legal text not required — passingThis template collects neither health nor regulated financial information.
  • Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.

Who this template is for

Guest Satisfaction Survey is built for product, support and customer-experience teams who need to measure satisfaction and capture the reason behind every score.

  • Product, support and customer-experience teams working in surveys & feedback.
  • Teams who need to measure satisfaction and capture the reason behind every score without writing code or paying for a custom build.
  • Anyone replacing ratings with no context, so nothing actionable comes out of them with one structured record per enquiry.
  • Respondents are customers, users and members — the form asks them 13 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of ratings with no context, so nothing actionable comes out of them and turns each submission into a record your team can act on immediately.

  • 7 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It pins the request to a specific date.
  • It scores satisfaction on a consistent scale.
  • It measures sentiment on a numeric scale.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “How likely are you to recommend us?” is greater than 8, the form dynamically exposes “Leave us a public review”. Otherwise that question never appears.
  • Features advanced show logic: if “How likely are you to recommend us?” is less than 7, the form dynamically exposes “What should we fix?”. Otherwise that question never appears.
  • Features conditional validation: if “How likely are you to recommend us?” is less than 7, “What should we fix?” becomes mandatory before the form can be sent.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

Public review scores are lagging indicators; this survey is the leading one. It scores the five things guests actually judge a property on — cleanliness, comfort, staff, check-in and value — separately, so a tired mattress does not get blamed on the receptionist. Asking for the room number is what turns feedback into maintenance: three low comfort scores from the same room is a work order, not a mood. The second page is where the survey earns its keep commercially. Guests who score nine or ten are shown a prompt to leave a public review, which is the legitimate way to raise a review average: you are asking everyone privately and inviting the happy ones publicly. Guests scoring six or below are required to say what should be fixed, and that answer reaches you before it reaches a review site. Send it the morning after checkout, while the stay is still specific.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 1 question. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “How likely are you to recommend us?

  • Reveals questions

If “How likely are you to recommend us?” is greater than 8, the form reveals “Leave us a public review”.

  • Reveals questions

If “How likely are you to recommend us?” is less than 7, the form reveals “What should we fix?”.

  • Makes answers required

If “How likely are you to recommend us?” is less than 7, “What should we fix?” becomes required before the form can be submitted.

Set it up step by step

  1. 1

Send it the morning after checkout

Detail fades fast; by day three you get "it was nice" instead of "the shower ran cold at seven".

  2. 2

Turn room numbers into work orders

Group low comfort and cleanliness scores by room and raise a maintenance job for the repeat offenders.

  3. 3

    Invite reviews only from promoters

Ask every guest privately and invite the nines and tens publicly — that is how a review average moves honestly.

  4. 4

    Reply to every detractor personally

A same-day manager email resolves most complaints before they are ever published.

Mistakes to avoid

  • Averaging all five ratings into one score, which hides the single fixable fault.
  • Skipping the room number, leaving maintenance with no idea which room to inspect.
  • Emailing a review-site link to everyone regardless of score, which imports complaints straight onto your public page.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Raise maintenance jobs from low-scoring rooms, reply personally to every detractor, and invite promoters to leave a public review.

Works in both layouts

These pages cover the same subject ground as the guest satisfaction survey, matched on the words people actually search for.

Frequently asked questions

Is inviting reviews from happy guests allowed?

Asking every guest for private feedback and inviting satisfied ones to review publicly is standard practice; never offer an incentive for a positive review.

How long should the survey be?

Should I ask for the room number?

Yes. It is the single most actionable field on the form and costs the guest three seconds.

What pairs with this?

The Guest Check-In Form before arrival, so pre-arrival, stay and post-stay all sit in one set of responses.
