The first page identifies the witness and their connection to the people involved, and records the practical detail that gives an account weight — the date and time of the event, where the witness was, how far away, what the light and weather were like, and whether anything blocked their view. The second page is the account itself: what they were doing beforehand, what they saw or heard in the order it happened, what anyone said, what happened immediately afterwards, and whether they spoke to anyone at the time. Two questions separate what the witness observed from what they were later told, which is the distinction that keeps a statement usable. It closes with a confirmation that the account is the witness's own and true as far as they know, plus a signature and date.
