About this template

Rescues need to place animals well and quickly, and this application form is written to surface the deal-breakers early so volunteers spend their time on realistic matches. Applicants give their contact details, age confirmation and the animal they are interested in, then answer a home section covering property type, garden and fencing, whether they own or rent, and landlord permission where relevant. The household block asks who lives there, the ages of any children, and which other pets are already in the home with their vaccination and neuter status. Experience questions cover previous ownership, what happened to those animals, hours the pet would be alone each day, and the plan for holidays and veterinary costs. Two references and the applicant's own vet are collected for checks. Because the form makes rental permission and lone-hours answers mandatory before submission, incomplete applications stop arriving altogether, and the whole file can be forwarded to a home-checker as a single record.
