A group class goes badly when one dog in the room should not have been there, and enrolment is where that is prevented. The first page covers the dog: name, breed, age, sex and neutering, weight, how long the owner has had them and where from, vaccination status with dates, whether they are on medication, and their vet practice. The second page is the part trainers actually rely on: how the dog behaves around other dogs on and off lead, around people and children, whether they have ever growled, snapped or bitten and in what circumstances, how they cope in a busy environment, what training they have already had and with whom, what equipment the handler uses, what motivates the dog, and any behaviour the owner finds difficult. It asks which class level the handler thinks fits, what they want to get out of it, and whether anyone in the household has a mobility consideration, because handling in a class involves the whole family.
