About this template

The booking is already made; this is the record staff work from while the pet is in their care. The first page confirms the stay and the pet: name, species and breed, drop-off and collection dates and times, the pet's temperament with staff and other animals, whether they can share a run or need to be alone, and how they usually cope with kennels. The second page is the operational detail that a booking form never captures. Feeding is recorded properly: what food, how much, how many times a day, at what times, whether food is provided by the owner, and any feeding routine such as slow bowls or separate feeding. Medication is recorded with name, dose, time of day, how it is given and where it is stored. It records the owner's vet and an authorisation limit for emergency treatment, a second contact who can make decisions if the owner is unreachable abroad, exercise needs, sleeping habits, toileting routine, what they have brought with them, and anything that helps them settle.
