About this template
This veterinary intake form is for clinics registering an animal they have not treated before, so the first consultation starts with a complete record instead of a clipboard scramble. It collects the owner's contact details and preferred way of being reached, then moves to the animal itself: species, breed, age, weight, sex, neuter status and microchip number. The medical section captures current medications, known allergies, vaccination status, previous surgeries, diet and behavioural warnings such as a tendency to bite when handled, which protects the nurse who takes the animal through to the exam room. A final block asks for the reason for today's visit, the referring practice if there is one, and consent to treat plus a spending limit for unforeseen work. On submission the record lands in your inbox and can be routed straight to the practice management system or printed for the patient file, so a receptionist only has to confirm details rather than type them.
